About VNA
VNA operates programs that offer results that positively impact the community and improve the lives of over 5,000 individuals each year. We serve the Montgomery County, Bucks, and Philadelphia Counties or Montgomery County, PA, and surrounding communities.

Our Mission
The mission of VNA is to support the integrity and independence of vulnerable individuals and families in the community. The agency’s goals are to identify and respond to the unmet health and social service needs, to work cooperatively in the greater community, to use resources wisely, and to make a clear and direct difference in the lives of people served with interventions that increase capacity, self-esteem, and self-sufficiency.

Primary Care Transition
VNA's programs serve all ages with health and human service interventions. For over 30 years, the VNA Children's Health Center has served as a patient-centered medical home for children who are uninsured or covered through Medical Assistance. We are now in the process of transforming this best-practice center into a family health center that will provide quality primary health care for the entire family.

Our History
VNA has been providing home and community health and social service solutions to area residents since 1919. The agency was founded to address public health concerns during the height of the 1918 Flu Pandemic. Since that time, the VNA has changed and expanded services to meet the growing needs of the individuals, families, and communities we serve.

In October 2021, the VNA became a subsidiary of Connexus Human Services Network. This partnership has initiated focused efforts to expand service offerings in the areas of primary health care for families, public benefit access, and in-home services for seniors.
