OBJECTIVE:

 

Lone Star Circle of Care is seeking a dedicated endocrinologist to establish specialty care servicing both our federally qualified health center (FQHC) patients and the broader community. The ideal candidate brings expertise in diagnosing and managing endocrine conditions while embracing a mission-driven, patient-centric approach to care. Responsible for the medical care of LSCC patients by providing comprehensive outpatient diagnosis and treatment of endocrine (hormone-related) conditions and disorders, such as: diabetes, thyroid dysfunction, obesity, osteoporosis, adrenal disorders, lipid disorders, metabolic abnormalities, growth disorders, or conditions of sexual function and reproduction.

Physicians are responsible for upholding the values and mission statement of LSCC and will conduct themselves as senior level, full time professional employees of the organization. Under the direction of the Medical Director, delivers primary care commensurate with training to patients of LSCC. Delivers care appropriate for an outpatient, primary care community health center with multicultural, multilingual patient population.

 

RESPONSIBILITIES:

 

Daily Duties:

 

  • Examines, diagnoses, treats and/or refers patients assigned to physician’s panel, on an outpatient basis. Treat and manage patients who present for care typically managed by an Endocrinologist. Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.   
  • Provide comprehensive outpatient endocrinology care for patients with diabetes, thyroid disorders, adrenal and pituitary diseases, osteoporosis, and metabolic disorders.
  • Diagnose and manage complex endocrine conditions, including Type 1 and Type 2 diabetes, gestational diabetes, thyroid nodules, polycystic ovarian syndrome (PCOS), and lipid disorders.
  • Develop personalized treatment plans, incorporating medication management, lifestyle modifications, and evidence-based interventions.
  • Perform and interpret diagnostic tests such as dynamic endocrine testing.
  • May perform and interpret thyroid ultrasounds and fine-needle aspiration biopsies.  
  • Collaborate with primary care providers, dietitians, diabetes educators, and clinical pharmacists to enhance chronic disease management.
  • Implement population health strategies to improve diabetes and metabolic disease outcomes in underserved populations.
  • Educate patients and families on self-management strategies, medication adherence, and lifestyle changes to prevent complications.
  • Engage in innovative care models to expand access to endocrinology services.
  • Support quality improvement initiatives, clinical research, and health equity efforts to address disparities in endocrine care.
  • Complete medical charts in a timely manner after each visit in the electronic medical records system
  • All other duties or responsibilities as assigned by Medical Director

 

Coaching/Training:

 

  • May provide in-service trainings for staff in primary care management of endocrine conditions
  • Occasionally participate in a high-quality educational experience for medical students, residents and other individuals in a training role

 

Operations:

 

  • Provide quality medical care according to standards established by LSCC
  • Assist and participates in internal quality control audits
  • Supervises nurses, medical assistants and lab personnel in area of specific support services for patient care delivery
  • Fulfill established guidelines set in place for provider encounter levels
  • Actively monitor and engaged in hitting productivity, patient satisfaction, and clinical quality metrics
  • Meet or exceed budgeted provider encounter targets
  • Attend internal and external meetings, workshops, conferences, and participate on task forces or committees, when necessary and as required

 

Compliance:

 

  • Perform all duties and services in full compliance with LSCC’s Quality Standards
  • Achieve and maintain LSCC Core Clinical Competencies
  • Apply for and sustain faculty appointment at Texas A&M University or other institutions as required
  • Physician will maintain all credentialing and licensing per LSCC policy
  • Physician will maintain MOC per ABIM/AOBIM guidelines and board certification while at LSCC.
  • Perform required monthly peer audits, if required
  • If supervising collaborating nurse practitioner(s)/physician assistant(s) in the specialty, have monthly meetings with these advanced practice providers

Maintain competencies, LMS trainings, BLS (AHA) as current

 

KNOWLEDGE, SKILLS AND ABILITIES:

 

  • Thorough knowledge of the principles and practices of medicine and specialty care (Endorcrinology) commensurate with his/her training.
    • In-depth understanding of diabetes, thyroid, adrenal, pituitary, osteoporosis, and metabolic disorders.
    • Familiarity with the latest clinical guidelines for endocrinology management, including insulin therapy, thyroid hormone replacement, and osteoporosis treatment.
    • Proficiency in interpreting hormonal assays, dynamic endocrine testing, continuous glucose monitoring (CGM) data, thyroid ultrasound, and bone density scans (DEXA).
    • Expertise in insulin pump therapy, CGM devices, GLP-1 receptor agonists, SGLT2 inhibitors, and lifestyle interventions for glycemic control.
    • Knowledge of thyroid nodules, hyperthyroidism, hypothyroidism, thyroid cancer, and calcium metabolism disorders.
    • Understanding of weight management strategies, including pharmacologic, lifestyle, and surgical interventions.
    • Knowledge of pharmacologic interventions, including diabetes medications, hormone replacement therapies, and osteoporosis treatments, including potential drug interactions and side effects.
  • Competency in performing thyroid ultrasound, fine-needle aspiration, insulin pump adjustments, and continuous glucose monitoring setup.
  • Capacity to educate primary care teams on best practices for diabetes and endocrine disease management.
  • Experience or interest in developing and implementing chronic disease management programs to improve health outcomes.
  • Generates and/or recommends medical protocols to govern the delivery of Endocrinology care throughout the Center that are consistent with standards of care.
  • Utilizes quality improvement tools to develop processes that promote continuous improvement in delivery of patient care, patient outcomes or workflows.
  • Ability to collaborate effectively with people of diverse races, ethnicities, nationalities, sexual orientations, gender identities, gender expression, socio-economic backgrounds, religions, ages, and English-speaking abilities
  • Ability to deal effectively with patients, medical staff, administration and the public
  • Excellent oral and written communication skills
  • Ability to recognize and understand the public health, social, and economic factors which impact the overall health of LSCC patients
  • Ability to support LSCC mission and readily serve its patient population
  • Maintain a professional manner and appearance that contributes to the professional image presented to individuals within and outside of LSCC
  • Ability to work independently with a high level of self-motivation
  • Ability to work in and support electronic health records environment

 

REQUIRED EXPERIENCE, EDUCATION, AND LICENSE:

 

  • Current valid license to practice medicine in the State of Texas
  • M.D. or D.O. degree from an accredited School of Medicine or Osteopathy and completion of an Internal Medicine residency program
  • Board certification in Internal Medicine is required
  • Board certification in Endocrinology is required
  • Current DEA certificate
  • Basic Life Support (BLS) certification from American Heart Association or American Red Cross
  • Maintain continuing education as required by licensing and any applicable accrediting boards, maintenance of certification if applicable, and completes additional educational units as required by LSCC (e.g., risk management courses)

 

PREFERRED EXPERIENCE, EDUCATION, AND LICENSE:

 

  • Two (2) years’ experience in an outpatient healthcare clinical setting in specialty (Endocrinology)
  • Prefer experience treating patients of all ages
  • Proficiency using electronic medical records (EMR); familiarity with telemedicine platforms and digital health tools
  • Proficiency with MS Office Suite (Word, Excel, and Outlook)
  • Bilingual English/Spanish language skills are preferred but not required
